Q: Does this have drill holes if I wanted to screw it in? Instead of stick on?

No it doesn't have screw holes, but the stick on works very well

Q: Does the end with the cable attached rotate away when not in use?

This lock does not rotate. It has very strong sticking to the side and on the door. You just use the key to unlock just as you would any other lock. Tried removing where it had been stuck on because the key got lost and there was no removing it. The cable had to be cut.

Q: I plan to use it for my wood door. Is it going to damage my door if I remove this? How easy it is to remove?

The glue should hold well enough to screw up your paint if you try and remove it.
